Summary
Seeking a highly motivated candidate to outreach to Medi-Cal members of HCLA in need of health care improvement interventions. Candidate must be able to motivate and guide patients to visit their Primary Care Physician (PCP) office. A goal of improving compliance with completion of these quality screening tests and exams is a crucial component of the position.
Duties and Responsibilities:
- Using several HCLA available HCLA databases, contact potential patients and educate members on the importance of scheduling appointments that will vastly improve their health status. There are approximately 15 health screening measures members may be eligible to complete within the year.
- Assist members in making their appointments.
- Address any barriers to accessing the care to ensuring members receive services in a timely manner.
- Improve patient health outcomes through member engagement.
- Complete a specified volume of outreach calls daily for assigned PCP Health Centers.
- Engage with Health Centers, health plans, and senior staff at HCLA, reporting findings on a regular basis.
- Document outreach activity including productivity (tracker templates) on a weekly, monthly and quarterly basis for HCLA leadership, health plans, etc.
- In collaboration with HCLA leadership, management staff and health plan representatives, document processes and workflows that are successful in completing health care gap closure screening visits.
Qualifications:
- Bilingual English and Spanish required
- High school diploma or equivalent required (Bachelor’s degree preferred)
- Proficient in Microsoft Office (Word, Excel, PowerPoint, Outlook) required
- 2-3 years of experience and knowledge of a managed care health care delivery system preferred
- Organized with excellent communication and writing skills
- Excellent Listening skills
- Understanding of medical terminology
- Compassionate and empathic to the needs of the patient community
